RESIDUAL VAPOURS IN TANKS/PIPELINES — Gas-free and certify; isolate/blank lines; ventilate; re-test at set intervals.

OXYGEN ENRICHMENT — Keep O₂ away from oils/grease; no O₂ for “ventilation”; control leaks; monitor atmosphere; remove contaminated clothing.

IGNITION SOURCES (SPARKS/ARC/SLAG) — Use spark/screens and fire blankets; wet down or shield below/adjacent; Exclusion zone; remove combustibles.

LET-GO AT EXCESSIVE SPEED — Reduce to bare steerage/stopped; head into wind/current; coordinate bridge–fo’c’sle; “walk out in gear” when required.

WRONG SCOPE OF CABLE — Veer 5–7 × water depth (more if exposed); adjust with conditions; mark shackles and record out.

DRAGGING ANCHOR — Veer more chain; use engines to relieve load; set second anchor if approved; keep anchor watch/alarms; be ready to heave up and proceed to sea.

BRAKE SLIP / NOT SET — Test brake at short stay; set properly; use bow stopper/guillotine; never rely on brake alone in heavy conditions.
